SAFY is seeking a full-time Case Manager I for our Louisville division in Kentucky.

As a Case Manager, you will be responsible for providing in-home support services for foster parents and children. This position is for our Therapeutic Foster Care program.

 What Youll Do 

• Work independently and create your own schedule based on your caseload and availability of your assigned families.
• Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 15-18 clients that youll see weekly. 
• Create documentation for all services you provide in our Electronic Record System.
• Participate in rotating on-call schedule with the other team members. Generally, we're on call 1 week every 2 months.
• Develop and maintain consultative and liaison relationships with DCBS and other members of the youths treatment team (biological parents, school, etc.)
• Collaborate with outside parties including biological families, schools, courts, parents, hospitals, doctors, etc.
• Travel to families homes in and around Louisville mileage reimbursement is included. Overall hours being worked outside of the office is estimated to be around 60%, which includes traveling to and from visits and time spent at the home conducting a home visit. 
• Set your schedule in collaboration with the team. Generally, we work standard hours of around 9am 6pm, however there are times when you may need to work until 8pm or later, based on the needs of families on your caseload.

Qualifications 

• Bachelor's degree in the field of social work, counseling, psychology, sociology, child & family studies, or other approved related field of human services.
• Two years of relevant experience in social work/adoption is preferred.
• Must possess a valid drivers license and reliable transportation in order to meet the travel requirements of this position.

 What We Offer 

• Targeted hiring range: $44,000.00 to $53,000.00
• Compensation is commensurate with transferrable experience, education, and licensure of candidate, location of the position, along with internal equity and budgeted amount for the role.
